is exactly what they do.

handle is what might work for you.  What about a D-handle like the ones used
on the old lever machines?  They were sort of a shovel handle shape, but
operated the lever.  They had a single threaded attachment point.  One of
those, along with a length of something between it and the portafilter could
give you the leverage and comfort you need; a D-handle for grip, and a few
inches of bakelight for a shaft to make the most of leverage.  I'm sure a
good machine shop could make you something as well.  I suppose you could
also find a cheap shovel with an appropriate handle (cheap because the
materials are likely to be lightweight), and take that to a machine shop and
ask them to have it matched and attached to your PF.
Another option would be to buy a handle used to make jigs for shops.
They're available with female ends; you could have a length of steel rod
bent to 90 degrees and threaded on both ends to match your portafilter and
your handle threads.  I don't think that would be very pretty, but it might
be a cheap experiment to find out if that orientation is better for you.

La Marzocco PF handles are downward angled. Not vertical downward, something
like 45 IIRC but maybe less angle. The ears need to be slightly "thinned"
(ground) to fit Silvia or E61. If you order from Espressoparts.com they'll
mod it for you.
If the handle was completely vertical, seems it would be difficult to get
enough pressure to lock in.

How about a steering wheel knob?  It would clamp on the end of your
portafilter handle and give you a ball to grab when tightening the handle.
Search "steering knob" on Amazon and you will see a variety, from about $10.
